Privacy Policy
Fibers Neurological Institute, LLC ("Fibers", "we", "us") is a neurosurgery practice in Florida. This policy explains what information we collect through our website, our patient intake and signing pages, our phone lines and our text messaging program, and how we use and protect it. Your medical information is also protected under the Health Insurance Portability and Accountability Act (HIPAA) and our Notice of Privacy Practices, which every patient receives at intake.
Information we collect
- Contact details you give us: name, date of birth, mailing address, email address and phone numbers.
- Appointment, intake and treatment information needed to provide care and to bill for it.
- Records of our communications with you, including texts, calls and emails.
- Basic technical information when you use our website, such as browser type and pages visited.
How we use it
- To schedule and confirm appointments, send you intake documents to review and sign, remind you of visits, and follow up on your care.
- To coordinate with your other providers, facilities, insurers and, when you have a personal injury case, your attorney, as permitted by HIPAA and your authorizations.
- To operate, secure and improve our services.
Text messaging (SMS, MMS and RCS)
If you give us your mobile number and agree to receive text messages, we send appointment confirmations and reminders, links to intake and consent documents, care instructions and follow-up messages, and replies to questions you text us. Message frequency varies with your care. Message and data rates may apply. Reply STOP at any time to stop receiving texts and HELP for help, or call us at (786) 607-5911.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties. Information may be shared with subcontractors that provide support services to us, such as our messaging platform provider, solely to deliver the messages you agreed to receive. All other categories of use exclude text messaging originator opt-in data and consent.
Sharing
We do not sell personal information. We share information only as needed to provide your care and run the practice: with the people and organizations involved in your treatment and payment, with service providers who work on our behalf under confidentiality obligations, and when the law requires it. Protected health information is shared only as HIPAA and our Notice of Privacy Practices allow. None of this sharing includes your mobile phone number or your text messaging opt-in data and consent: those are never shared with, sold or rented to third parties or affiliates for marketing or promotional purposes.
Security and retention
We protect information with administrative, technical and physical safeguards, including encrypted storage and transmission and access limited to staff who need it. Medical records are kept for the period Florida law requires.
Your choices and rights
You may opt out of text messages at any time by replying STOP. You may ask to see or correct your records, request restrictions on how they are used, and ask for a copy of our Notice of Privacy Practices by contacting us.
